JOB SUMMARY:

Under the general supervision of the Program Director or Designee, functions as part of a team providing client-centered, strength-based rehabilitation and recovery services to assigned consumers that relate to the care, nurturing, treatment, education, socialization, habilitation, recovery/healing and support of adults diagnosed with a mental illness and in some instances, co-occurring substance abuse disorder. These tasks focus on supporting, instructing and assisting recipients of mental health services to develop the skills needed to attain personal goals, live, be educated, work, and socialize successfully in the community environments of their choice.
Duties:The program Case Manager will have the prime responsibility for organizing comprehensive service planning and assessment activity and assuring service plan implementation through direct case management or co-provider services. The Case Manager will also have either primary or shared responsibility, depending upon the specialty/discipline, in providing the residents with specific linkages/advocacy services with co-providers of housing, mental health, substance abuse, and healthcare services etc. as well as with parole boards. Together with the residents, the Case Manager will formulate residentsí comprehensive service plan in conjunction with specialty program staff. For some residents the Case Manager will be the major provider of a particular service and, for others, a secondary support agent.

Qualifications:MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:

High School diploma or GED plus a minimum of 2 years experience providing direct services to individuals with mental disabilities ($24,500); associate's degree plus 1 year experience as stated above ($26,500); bachelor's degree plus 1 year experience as stated above ($30,000) or BSW and no experience ($30,000). Requires very good organizational, computer and writing skills.
