Client Care Manager

Description:Serve as a liaison between clients and clinical staff to ensure that client’s needs are met.
Duties: Serve as liaison between clients and clinical staff.

Monitor the implementation of the care plan to ensure that programs are followed

Conduct intake interview, orientation of clients to facility rules and regulations

Coordinate schedules with other professional services and care teams

Assist clients with adjustment to shelter living with periodic follow-up

Motivate the client towards the goal of permanent housing

Resolve client inquires and disputes

Conduct substance abuse groups and workshops for clients

Maintain log book documenting all activities in the shelter on the assigned shift and noting any variance to the normal day to day operation

Maintain head count; monitor the census and placement of clients to assure compliance with DHS rules and regulations

Plan and organize field trips, house meetings, and special activities for residents and staff

Monitor the physical upkeep of the facility

Manage and oversee all aspects of the facility in the absence of the Program Director and/ or Social Services Director

Oversee Supervise Residential Aides, Maintenance staff, Food Manager, and Nurse’s aides on assigned shifts
Qualifications:Bachelor's degree, preferably in a human service field. A minimum of two years experience providing services to people with mental disabilities and/or substance abuse disorder and/or homelessness, or closely related experience.
